Title: “Retina Buttonholed”

Description: It’s an OCT depicting one of the patterns of intraretinal fluid progression in a case of Optic disc pit. 21 years old girl diagnosed as ‘Karnenburg syndrome bei grubenpapille’ in Germany presented to us for surgical intervention, as already advised there. Examination at presentation, revealed optic disc pit with retinoschisis, intraretinal and subretinal fluid(SRF) with SRF being the predominant fluid type. SRF, surprisingly showed very fast progression to more than 1600 microns from 1049 microns accompanied with painless drop of VA from 6/60 to 3/60 over two weeks. Fundus fluorescein angiography performed to rule out other possibilities. The scan has been taken at the edge of fast progressing fluid over two weeks , and observes one of the ways of fluid progression in Optic disc pit that leads to schisis prior to SRF being captured as a ‘Button hole’ appearance.
