Title: Frozen Flow and a Starry Glow
Description: A 43 year old hypertensive male presented with sudden onset defective vision in left eye of 2 days duration. His vision in RE was 6/6 and LE was CF2m. Anterior segment was WNL. Fundus showed a large infarct superonasally with disc edema and peripapillary hemorrhages along with macular edema which was confirmed on OCT. We suspected a combined occlusion and started him on systemic steroids after medical clearance. 2 weeks later his vision improved and disc and macular edema also subsided revealing a macular star. Blood investigations to rule out infective causes were negative and coagulation profile was normal. FFA showed only a mild disc leakage in the left eye.He was followed up with tapering doses of steroids and vision improved to 6/9 in the left eye.
We concluded it was an atypical presentation of Neuroretinitis with Branch retinal artery occlusion.
Case reports of B.Henselae neuroretinitis presenting with BRAO have been reported.
